Modification
Booking of Services - Age 18 or Older
You understand and agree that you must be 18 years of age or older to book any services or have your parent or guardian’s written consent. Only those who are 18 years old are permitted to register an account for online booking.

Purpose of the Website
The purpose of this Website is to allow current and future clients of Direct Aesthetics, PLLC to obtain additional information about Direct Aesthetics, PLLC services and products, book appointments, purchase products and to contact Direct Aesthetics, PLLC with any questions or comments that you may have. Any other use of this Website is prohibited.

Communications
By creating an Account on our Website, you agree to subscribe to newsletters, marketing or promotional materials and other information we may send. However, you may opt out of receiving any, or all, of these communications from us by following the unsubscribe link or instructions provided in any email we send.

If you wish to purchase any product or service made available through the Website (“Purchase”), you may be asked to supply certain information relevant to your Purchase including, without limitation, your credit card number, the expiration date of your credit card, your billing address, and your shipping information.

Fee Changes
- Direct Aesthetics, PLLC., in its sole discretion and at any time, may modify the Subscription fees for the Subscriptions. Any Subscription fee change will become effective at the end of the then-current Billing Cycle.
- Direct Aesthetics, PLLC will provide you with a reasonable prior notice of any change in Subscription fees to give you an opportunity to terminate your Subscription before such change becomes effective.
Your continued use of the Website after the Subscription fee change comes into effect constitutes your agreement to pay the modified Subscription fee amount.
Accounts
When you create an account with us, you guarantee that you are above the age of 18, and that the information you provide us is accurate, complete, and current at all times. Inaccurate, incomplete, or obsolete information may result in the immediate termination of your account on the Website.
You are responsible for maintaining the confidentiality of your account and password, including but not limited to the restriction of access to your computer and/or account. You agree to accept responsibility for any and all activities or actions that occur under your account and/or password, whether your password is with our Website or a third-party service. You must notify us immediately upon becoming aware of any breach of security or unauthorized use of your account.
You may not use as a username the name of another person or entity or that is not lawfully available for use, a name or trademark that is subject to any rights of another person or entity other than you, without appropriate authorization. You may not use as a username any name that is offensive, vulgar or obscene. We reserve the right to refuse service, terminate accounts, remove or edit content, or cancel orders in our sole discretion.
Termination
We may terminate or suspend your account and bar access to the Website immediately, without prior notice or liability, under our sole discretion, for any reason whatsoever and without limitation, including but not limited to a breach of the Terms.
If you wish to terminate your account, you may simply discontinue using the Website.
All provisions of the Terms which by their nature should survive termination shall survive termination, including, without limitation, ownership provisions, warranty disclaimers, indemnity and limitations of liability.

ARTICLE IV MERCHANDISE AND PRODUCT RETURN POLICIES; GIFT CARDS, BOOKING APPOINTMENTS AND CANCELLATION POLICY
NO RETURNS OR EXCHANGES
UNLESS OTHERWISE STATED, ANY PURCHASE OF MERCHANDISE, PRODUCTS OR GIFT CARDS FROM DIRECT AESTHETICS, PLLC SHALL BE NONREFUNDABLE AND NONEXCHANGEABLE.
Third-Party Sellers
The Direct Aesthetics, PLLC Website may include links to third-party manufacturers or sellers only as a courtesy to users. To the extent that the Website includes links to other websites for the sale of merchandise, products or other goods unaffiliated with Direct Aesthetics, PLLC, the third-party manufacturer or seller’s terms and conditions, including any warranties, disclaimers or return policies, shall apply. You understand and agree that Direct Aesthetics, PLLC is not responsible for any claims or causes of action concerning services or products provided by third-party manufacturers or sellers.
Gift Card Policy
Direct Aesthetics, PLLC offers gift cards for purchase on the Website. The purchase of gift cards will utilize a third-party payment processor, as stated in Article V. GIFT CARDS MUST BE USED WITHIN FIVE (5) YEARS OF THE PURCHASE DATE OR THEY WILL NOT BE HONORED. GIFT CARDS HAVE NO MONETARY VALUE AND SHALL NOT BE PAID OUT IN THE EVENT THAT THE GIFT CARD IS NOT USED.
Booking Appointments
Direct Aesthetics, PLLC offers online booking of appointments via a third-party provider website. Online booking of appointments shall be on a first-come, first-served basis.
Cancellation Policy
You understand and agree that if you need to cancel an appointment after you have booked it, you MUST cancel at least 24 hours prior to the appointment. If a patient fails to show up for their appointment without prior notice, they will be charged $75. For group appointments, there is a 72-hour cancellation policy, and the credit card on file will be charged $200 if canceled less than 72 hours in advance.
Individual Appointments
YOU UNDERSTAND AND AGREE THAT ANY CANCELLATION LESS 24 HOURS OR LESS BEFORE THE SCHEDULED APPOINTMENT SHALL BE SUBJECT TO A NON-REFUNDABLE CANCELLATION FEE OF TWO HUNDRED ($75.00) DOLLARS, WHICH WILL BE CHARGED TO THE CREDIT CARD YOU HAVE ON FILE.
Group Appointments
YOU UNDERSTAND AND AGREE THAT ANY CANCELLATION LESS 72 HOURS OR LESS BEFORE THE SCHEDULED APPOINTMENT SHALL BE SUBJECT TO A NON-REFUNDABLE CANCELLATION FEE OF TWO HUNDRED ($200.00) DOLLARS, WHICH WILL BE CHARGED TO THE CREDIT CARD YOU HAVE ON FILE.
ARTICLE V THIRD-PARTY LINKS AND PROVIDERS
Certain third-party websites are included as links within the Website for specific purposes including the purchase of products, gift cards and other merchandise directly from Direct Aesthetics, PLLC, the purchase of products manufactured and sold by third-party suppliers, and the creation and registration of an account through a third-party vendor, which allows you to book appointments for services with Direct Aesthetics, PLLC.
Use of Third-Party Vendor Booking System
The booking system is a website that is created, operated and maintained by a third-party provider, which allows you to create and register an online account for purposes of booking appointments for services to be provided by Direct Aesthetics, PLLC (the “Booking System”).
Currently, the Booking System utilized by Direct Aesthetics, PLLC is Aesthetic Record, an unaffiliated third-party vendor that provides booking management software and systems. In order to utilize the Booking System, you will be redirected to the Aesthetic Record website.
You will be required to submit to the third-party your personal information including, without limitation, your full name, date of birth, address, telephone number, and payment information in order to create and maintain the account and book services online through the third-party vendor’s website.
It is important that you familiarize yourself with the terms of use or service for the third-party vendor’s website and ensure that you check back often for updates. By creating and maintaining an account through the Booking System, you are agreeing to the terms of use or service and the privacy policy of the third-party vendor and its website.

If you do not wish to create an account or do not agree to the terms of use or service or the privacy policy, you may book any services directly by contacting Direct Aesthetics, PLLC via telephone. Direct Aesthetics, PLLC reserves the right to change third-party vendors, at any time, without notice.
Use of Third-Party Payment Processors
Certain transactions involving the sale of products, gift cards and other merchandise sold directly by Direct Aesthetics, PLLC will involve the use of a third-party payment processor (the “Payment Processor”).
In order to complete a transaction, you will be required to submit certain personal information to the Payment Processor including, but not limited to, your name, billing and shipping address, telephone number, email address, credit card information and related information necessary to process the transaction. You understand and agree that Direct Aesthetics, PLLC makes no representations that the materials or content contained herein, including services or products offered, are appropriate or available for use outside the United States.
